What are These Conditions?
The cricopharyngeus is a small but critical muscle at the top of the esophagus that acts as a valve — relaxing to allow food and liquid to pass during swallowing, then closing again afterward. When this muscle becomes overactive, stiff, or fails to relax properly, swallowing becomes difficult and uncomfortable — a condition known as cricopharyngeal dysfunction (CPD).
Over time, the increased pressure generated against a poorly relaxing cricopharyngeus can cause the lining of the throat to herniate outward, forming a pouch known as a Zenker's diverticulum. Food and liquid can collect in this pouch, leading to a distinctive and often distressing set of symptoms. While CPD and Zenker's diverticulum are distinct conditions, they share a common underlying mechanism and are closely related.

Common Symptoms
- Difficulty swallowing solids, liquids, or both
- The sensation of food sticking in the throat
- Regurgitation of undigested food, sometimes hours after eating
- Gurgling sounds in the throat, particularly after meals
- Chronic cough, throat clearing, or halitosis
- Unexplained weight loss due to difficulty eating
Diagnosis & Treatment

Treatment depends on the severity of symptoms and the specific diagnosis. For cricopharyngeal dysfunction, options include balloon dilation of the cricopharyngeus muscle — particularly useful in cases where radiation therapy has caused scarring or stiffening of the muscle — or endoscopic laser cricopharyngeal myotomy, a precise surgical technique to permanently release the muscle. For Zenker's diverticulum, endoscopic laser diverticulotomy addresses both the pouch and the underlying muscle dysfunction in a single, minimally invasive procedure — without the need for an external incision.
