What is a Goiter?
A goiter is an abnormal enlargement of the thyroid gland. It is not a diagnosis in itself, but rather a finding that can result from a variety of underlying conditions — including thyroid nodules, hyperthyroidism, hypothyroidism, autoimmune thyroid disease, or iodine deficiency. Goiters range widely in size and severity, from a barely noticeable fullness in the neck to a substantial enlargement that compresses surrounding structures and significantly affects quality of life.
While many goiters require only monitoring and medical management, those that grow large enough to cause compressive symptoms — or that harbor suspicious nodules — often require surgical treatment.
Common Symptoms of Goiter
- A visible or palpable fullness or swelling in the front of the neck
- A sensation of pressure, tightness, or fullness in the throat
- Difficulty swallowing, particularly with solid foods
- Shortness of breath or difficulty breathing, especially when lying flat
- Hoarseness or voice change due to pressure on surrounding structures
- A feeling that a shirt collar or neckwear has become too tight
Diagnosis & Treatment
Evaluation of a goiter begins with a thorough clinical examination, thyroid function testing, and ultrasound imaging to assess the size, composition, and characteristics of the gland. In some cases, additional imaging such as a CT scan may be recommended — particularly for large or substernal goiters that extend below the collarbone.
